Extended Warranty must be purchased within 45 days from the date of machine purchase.
A digital copy of the purchase invoice, receipt, or proof of purchase must be attached and submitted with this form.

Once submitted, a Ilco Customer Service Representative will contact you to finalize the purchase of the extended warranty.
